Hyaluronic Acid Fillers vs Sculptra: Long-Term Results Compared
If you are noticing deeper lines, hollow cheeks or a tired, drawn look and you are choosing between hyaluronic acid fillers and Sculptra, the main difference for long-term results is this: hyaluronic acid fillers give immediate, sculpted volume that typically lasts 9 to 18 months, while Sculptra works more slowly to stimulate your own collagen for results that can last 2 to 3 years. At Alcove Aesthetics by Dr Goziem on Pall Mall, just moments from Mayfair and Westminster, I often combine both, depending on whether you want instant lift, gradual regeneration, or a mix of the two.
Hyaluronic acid fillers are ideal when you want precise contouring of lips, cheeks, jawline and under-eyes with results that can be adjusted or dissolved if needed. Sculptra, by contrast, is best for global volume loss, laxity and skin quality in the mid to lower face, temples and jawline, particularly if you prefer very subtle, progressive change. Both are doctor-led injectable treatments, but they behave quite differently in the skin over time.
This guide breaks down exactly how each works, how long you can realistically expect results to last, who each option is best for, and how I approach treatment planning at my Central London facial aesthetics clinic to achieve natural, age-appropriate results that still look like you.
Introduction: Why women in their 30s, 40s and 50s compare these two treatments
Many of my patients in Mayfair, St James’s, Belgravia and neighbouring areas come to their consultation saying the same thing: “I don’t want to look ‘done’. I just want to look fresher, less tired, and more like myself again.”
From your mid-30s onwards, you naturally lose collagen, bone and fat in the face. This can lead to:
- Flattened or sagging cheeks
- Deepening nasolabial folds and marionette lines
- Hollow temples or under-eyes
- Looser skin and jowls along the jawline
Both hyaluronic acid fillers and Sculptra can address these concerns, but they do so in very different ways. Choosing the right approach is less about the product name and more about your anatomy, lifestyle, tolerance for downtime and how you want to age over the next decade.
What It Is & How It Works
How hyaluronic acid fillers work
Hyaluronic acid (HA) is a sugar molecule that naturally exists in your skin, joints and eyes. It binds water and gives the skin its plump, hydrated quality.
In aesthetics, HA is cross-linked into a gel and injected to:
- Replace lost volume (cheeks, chin, lips, temples)
- Soften lines and folds (nose-to-mouth lines, corners of the mouth)
- Refine structure and contour (jawline, nose, chin)
At Alcove Aesthetics, we use premium, CE-marked Dermal Fillers placed precisely in the correct layer of tissue. The results are:
- Immediate – you walk out seeing a visible change
- Predictable – we can sculpt millimetre by millimetre
- Adjustable – small tweaks can be made at review
- Reversible – if required, a dissolving enzyme (hyaluronidase) can break down the filler
Over time, the HA gradually breaks down and is safely absorbed by the body. Depending on the area treated, product chosen and your metabolism, this usually takes 9 to 18 months.
How Sculptra works
Sculptra is not a filler in the traditional sense. It is a collagen-stimulating injectable made from poly-L-lactic acid (PLLA), a biocompatible material that has been used in medicine for decades. Rather than acting as a gel that holds shape, it acts as a trigger for your fibroblasts to make new collagen.
Here is what happens after Sculptra is injected:
- A water-based solution containing tiny PLLA particles is placed in areas of volume loss (for example, cheeks, temples, jawline, lower face).
- The water is absorbed over a few days, so any initial “plumpness” you see at first will reduce.
- Your body recognises the PLLA particles and begins a controlled healing response.
- Over 3 to 6 months, new collagen is laid down around these particles, gradually restoring lost structure and firmness.
- The PLLA itself is slowly metabolised and cleared, leaving behind your own collagen network.
The result is a quiet, steady thickening and lifting of the skin and deeper tissues. At my Mayfair-adjacent aesthetic clinic, I most often use Sculptra for:
- Global mid-face and lower-face volume restoration
- Skin crepiness in the lower cheeks
- Early jowling and softening of jawline definition
- Long-term collagen support in patients committed to graceful ageing
Because the mechanism is regenerative, results tend to last longer than HA fillers, but they also require more patience and a carefully staged treatment plan.
Benefits & Results
The table below summarises the key differences in long-term outcomes between hyaluronic acid fillers and Sculptra.
| Feature / Outcome | Hyaluronic Acid Fillers | Sculptra |
|————————————-|—————————————————-|—————————————————-|
| Main action | Adds instant volume and definition | Stimulates your own collagen over time |
| Onset of results | Immediate (with some settling over 1 to 2 weeks) | Gradual, typically from 6 to 12 weeks |
| Typical longevity | 9 to 18 months (area and product dependent) | Around 2 to 3 years after full treatment series |
| Best for | Lips, tear troughs, cheeks, chin, jawline contour | Cheeks, temples, lower face, overall firmness |
| Level of control | Very precise, sculpted, reversible | Subtle, diffuse, not reversible |
| Look and feel | Soft, hydrated, structured where needed | Natural, “quiet” rejuvenation, improved skin feel |
| Sessions required | Often 1 main visit + review/tweak | Usually 2 to 3 sessions, 4 to 8 weeks apart |

Treatment Journey
1. Consultation and assessment
Your journey begins with an in-depth consultation at our Central London clinic. We discuss:
- Your concerns: for example, hollowness, lines, “tired” or “sunken” appearance
- Medical history, previous treatments and lifestyle factors
- How you want to look over the next 5 to 10 years, not just in 3 months
I assess your:
- Bone structure and fat distribution
- Skin quality and elasticity
- Asymmetries and muscle activity
From here I will explain clearly whether hyaluronic acid filler, Sculptra, or a combination plan will serve you best. Sometimes an alternative such as PRP, skin boosters or energy-based treatments may be more appropriate, and I will always tell you this honestly.
2. Your treatment plan
We create a personalised plan which may include:
- HA fillers for structure and precise contouring (cheeks, jawline, chin, lips, tear troughs)
- Sculptra for long-term volume and collagen support, especially in the mid and lower face
- Skin quality treatments (such as microneedling, peels or skin boosters) to enhance texture and glow
For filler appointments, we typically allow 60 to 90 minutes; for Sculptra, 45 to 60 minutes.
3. On the day of treatment
- I take clinical photographs for your confidential record and to help track your progress.
- The skin is cleansed thoroughly and numbing cream can be applied for comfort.
- For HA fillers, I use needles or cannulas depending on the area and technique required.
- For Sculptra, the product is carefully placed using specific injection patterns designed to promote even collagen production.
- You can watch the process or simply relax – I talk you through each step if you prefer.
You will see an immediate difference with HA fillers. With Sculptra, you will see some fullness from the water that then settles over a few days, before the gradual collagen-building effects begin.
4. Review and maintenance
We usually schedule a review:
- 2 to 4 weeks after HA filler to check symmetry and integration
- 6 to 8 weeks after Sculptra to assess response and decide on the next session
Long-term, many patients prefer a subtle maintenance plan every 12 to 24 months, rather than dramatic, irregular changes.
Recovery & Aftercare
Both hyaluronic acid fillers and Sculptra are injectable treatments, so some short-term effects are expected:
- Mild redness, swelling or tenderness at injection sites
- Occasional small bruises
- A feeling of “fullness” or tightness in the treated area initially
Most people feel comfortable returning to work or social commitments the same or next day, especially at our discreet Pall Mall location.
General aftercare includes:
- Avoid intense exercise, saunas and steam rooms for 24 to 48 hours
- Keep the area clean and avoid heavy makeup on the day of treatment
- Sleep on your back the first night where possible
- With Sculptra, you may be advised to perform gentle massage at home as directed
I provide tailored written aftercare for every patient and offer follow-up support if you have any questions or concerns.
Cost & Transparency
At Alcove Aesthetics, treatment plans are transparent and always discussed before you decide to go ahead.
- Sculptra: from £350 for 1 vial, £650 for 2 vials, £1,200 for 4 vials
- Dermal filler packages: from £650 for 2 ml (Precision) to £1,500 for 5 ml (Elevate)
- Combined and full-face rejuvenation plans are priced according to the volume and number of sessions required
A £50 booking fee secures your appointment and is redeemable against treatment on the day. We operate a 48-hour reschedule policy to keep appointments available and fair for all patients.
Because no two faces are the same, a full consultation is essential to provide an accurate quote and realistic expectations.
FAQs: Hyaluronic Acid Fillers vs Sculptra
Which lasts longer: hyaluronic acid fillers or Sculptra?
Sculptra typically lasts longer. Once your course is complete, results can last around 2 to 3 years, whereas most hyaluronic acid fillers last 9 to 18 months. Longevity depends on your metabolism, lifestyle, area treated and how much product is used.
Are the results from Sculptra as noticeable as filler?
They are noticeable but more subtle. Hyaluronic acid fillers create instant, clearly visible changes in shape and contour. Sculptra works quietly in the background, gradually improving volume and skin firmness. It is excellent if you want friends to think you look well-rested, not obviously “treated”.
Is Sculptra safer than filler?
Both treatments have very good safety profiles when performed by an experienced, medically qualified injector. The main difference is reversibility – HA filler can be dissolved with a specific enzyme, while Sculptra cannot. This is why careful assessment, correct indications and conservative dosing are essential.
Does it hurt?
Most patients describe discomfort as mild and very tolerable. I use topical anaesthetic cream, and many HA fillers also contain lidocaine for added comfort. For Sculptra, the solution can be mixed with local anaesthetic to reduce stinging during injection.
Who is not suitable for Sculptra or filler?
You may not be suitable if you are pregnant or breastfeeding, have certain autoimmune or bleeding disorders, active skin infection, or a history of specific allergies. Some patients with very thin skin or certain medical conditions may be better served by alternative treatments such as Microneedling or skin boosters. A full medical consultation with me is required to confirm suitability.
To explore whether hyaluronic acid fillers, Sculptra, or a bespoke combination plan is right for you, I recommend a detailed, face-to-face assessment. Individual results will always vary, and long-term outcomes are best when we work together on a thoughtful, staged approach rather than quick fixes.
